Man Claims Family Member Used His Identity to Seek Treatment: Police Blotter

SS Card, Birth Certificate Lifted at Coffee Shop
Someone took a man's document holder while he was at a coffee shop on the 500 block of East Ogden Avenue. The holder was stolen sometime between 11 a.m. and 2 p.m., according to police. The man realized he did not have the document holder when he got to his car. He returned to his table and could not find it. The holder contained a social security card, birth certificate and credit cards. A case is pending.TUESDAY, SEPT. 3
Package Stolen From Front Porch
Someone took a package from a front porch on the 5600 block of South Park Avenue. The incident occurred sometime between 2:30 p.m. and 4:35 p.m., according to police. The package was valued at $68.50 and will be replaced by UPS. A case is pending.Identity Theft
A man reported a family member used his identity to seek medical treatment. The man received billing statements totaling $3,212 in services he did not receive.
