Birth Certificate

A certified copy (with the seal of county clerk) of your child’s birth certificate. If that is not available, other governmental documentation (passport, naturalization papers, etc.) of your child’s identity will be acceptable if accompanied by an affidavit explaining the inability to produce a copy of the birth certificate.

Proof of Residency

Three documents are needed as follows:

One of the following:

  • The most recent real estate tax bill for your residence showing you as the taxpayer. If your residence was purchased within the previous year, you may submit a home ownership title, deed, or closing statement for the purchase of your residence in lieu of a real estate tax bill.
  • Signed lease for your residence
  • A notarized letter from the owner of your residence stating that you reside at that residence and the duration of residency.

Two of the following:

  • Current gas, electric or water bill
  • Public Aid card
  • Voter registration with current address
  • Current home/apartment insurance certificate
  • Automobile registration from the State of Illinois
  • Land line telephone bill or recent cable TV bill
  • Current receipt for city vehicle sticker.

Household Information

  • Address and phone numbers
  • Parent contact information—work and cell phone numbers, email addresses and work schedules
  • Student information—demographic and health/medication information
  • Emergency contact information—phone numbers
  • Credit card or e-check payment information.

Children must be registered for enrollment in their attendance area school. Parents interested in enrolling their child in a District 181 school outside their neighborhood attendance area must first register at their home school, as well as indicate their preference in a letter to the principal of the preferred school. The Superintendent will make the final decision as to whether there is space available for non-attendance area students. Parents must provide transportation for students attending non-neighborhood schools.
