District 181 Offers Sports Physicals August 14

Seventh and eighth grade students attending Hinsdale Middle School or Clarendon Hills Middle School will have the opportunity to participate in sports physicals on Wednesday, August 14, 2013, at Clarendon Hills Middle School, 301 Chicago Avenue, Clarendon Hills. Students in seventh and eighth grade who plan to take part in pom pons, cheerleading, cross county, basketball, volleyball, or track must have a sports physical on file in the nurse’s office before team try-outs or interscholastic athletic events. A completed sports physical examination will be valid for all sports for a period of one calendar year. Dr. Harazin and Associates will be administering the sports physicals for a $20 fee.

The following are important details for parents who would like their students to participate in the August 14 sports physical exam:

  • The sports physical exam form is available on the District 181 website at www.d181.org > Resources > Forms.
  • The medical history component of the sports physical form must be completed and signed by the parent prior to the examination.
  • Sports physical examinations are completed on a first-come, first-serve basis.
  • Boys should arrive at 12:00 p.m.; girls should arrive at 1:00 p.m.
  • Checks should be made payable to Dr. Harazin.
  • The doctor requests that girls wear a one-piece swimsuit under shorts and a t-shirt. Boys should wear shorts and a t-shirt. Girls and boys should wear gym shoes.
